PLEASE NOTE:  All Entry Forms must go via BWFSC Coaches Secretary, Denise Whyman and returned by the closing dates as set by the Coaches Secretary.. 

Incomplete forms will not be accepted.  

All members are expected to keep a record of their pbs for inclusion on gala entry forms.

All forms  must be completed in full, ie, name, date of birth, contact details and ASA Registration number together with a cheque (or cash) made payable to BWFSC.  If you are unsure of the ASA Registration Number click here
